An approved HIPAA authorization will be accepted by most medical providers in the State of California. However, some, such as U. C. Davis Medical Center, Sacramento & St. Joseph's Behavioral Medical Center, Stockton require their own.
If you desire mental health or psychological records, you must modify the HIPAA authorization to reflect your request. If it is not done correctly, it will be rejected by the provider.
Copy of HIPAA authorization can be used for both psych and non-psych records.
Copy of V.A. authorization can be used or you can use a HIPAA authorization above.
Copy of SFFD authorization can be downloaded, printed and then faxed or mailed to Legal Beagle.
